  • To obtain a copy of your adjudicated report (closed or dismissed), or a letter of confirmation for lost, damaged, or stolen property on open cases, please complete the electronic Police Records Request Form.

Note: This form is not intended to be used for Freedom of Information Act (FOIA) requests. For FOIA requests please visit https://foia.msu.edu.

  • To request personal background checks and a letter of clearance, please complete the electronic Personal Background Request Form

For documentation of car crashes and accidents on public roadways, MSU completes a State of Michigan UD10 Traffic Crash Report form and submits the data to the State of Michigan. Insurance companies typically request a copy of the crash report if you have been involved in a traffic crash.

There are two ways to obtain a copy of your traffic crash report.

  • Reports can also be requested and are available immediately once order is submitted to the Michigan Traffic Crash Purchasing System (TCPS).  Please allow a few days for report to be completed after the accident.
  • To obtain a copy of a private property accident or public roadway accident directly from MSU Police and Public Safety, complete the electronic Traffic Request Form.
